👋⭐️Let’s talk Osteoarthritis
    ⁃    Osteoarthritis is a result of wear and tear over the years and most commonly occurs in patients over 50 years of age. The cartilage around the knee becomes damaged and is gradually worn down resulting in pain and stiffness.

⭐️What are the signs & symptoms of arthritis?
    ⁃    Severe pain that limits your activities of daily living (walking, climbing stairs, getting up from a chair, participating in sports)
    ⁃    Moderate to severe pain that occurs during rest or awakens you at night

⭐️What are the treatment options?
    ⁃    Non surgical: physical therapy, weight loss (if overweight), anti inflammatory medications, injections, and bracing
    ⁃    Surgical: knee replacement

⭐️ Kinematic Knee Replacement
    ⁃    Total knee replacements can be performed using either Mechanical Alignment (MA) or Kinematic Alignment (KA). Traditionally, most knee replacements have followed the mechanical alignment method, in which every patient’s knee is positioned using the same standardized angles to create a perfectly straight leg. However, we now understand that no two knees are exactly alike. Many individuals naturally have a slight varus (bow-legged) or valgus (knock-kneed) alignment, and their ligaments, soft tissues, and patella (kneecap) are adapted to function in those native positions.

    ⁃    Research now shows that up to 20% of patients with a mechanically aligned knee replacement may continue to experience pain, stiffness, or instability.

    ⁃    In response to these outcomes, and backed by decades of research, more surgeons are now adopting kinematic alignment. This approach customizes the placement of the implant to match the patient’s original, pre-arthritic anatomy and alignment. The result is a more natural-feeling knee optimized for comfort, stability, and overall function.

    ⁃    Dr. Almaguer uses this patient specific approach with kinematic alignment so each patient gets a truly custom knee replacement that matches their individual anatomy.

    ⁃    Additionally, Dr. Almaguer offers the use of new implant technologies such as the Persona IQ smart implant and 3D printed cementless implants.

Beyond performing surgery, one of my most important roles is to serve as an educator. As your surgeon, my responsibility is not only to provide an accurate diagnosis but also to help you understand that diagnosis and the full range of treatment options available. In many cases, symptoms can be effectively managed through conservative, non-surgical approaches, which may delay or even eliminate the need for surgery. I always emphasize to my patients that surgery is a last resort—I never want to perform an operation unless it is absolutely necessary. 👋 Let’s talk hip dysplasia

⭐️What is hip dysplasia?

-Hip dysplasia occurs when a shallow acetabulum (hip socket) provides decreased coverage for the femoral head (ball). Over time this decreased coverage loads the edge of the acetabulum, resulting in labrum tears, and eventually cartilage wear.
-Hip dysplasia is the second most common cause of early onset arthritis in the United States. 🇺🇸
-Most people are aware of hip dysplasia in infants but mild dysplasia is often not diagnosed until early adulthood when symptoms first present.
-Hip dysplasia is more commonly found in females, particularly athletes involved in sports that require a wide range of motion (ballet 🩰, gymnastics 🤸‍♀️, figure skating ⛸️, military training 🪖).

⭐️What are the signs & symptoms of dysplasia?

- Hip or groin pain, clicking or popping 💥 , or a sensation of instability or shifting in the joint.
- If you have any of these symptoms it is important to be evaluated by a hip preservation specialist to be sure the subtle clinical and X-ray findings aren’t missed.

⭐️What are the treatment options?

- Non surgical: home exercise programs, guided physical therapy, NSAIDS, injections (steroid or platelet rich plasma)
- Surgical: combined hip arthroscopy with labral repair and periacetabular osteotomy
- Improper treatment with hip arthroscopy alone has up to a 90% failure in one year.

⭐️ Periacetabular Osteotomy (PAO)

- Peri (around) acetabular (hip socket) osteotomy (cut bone) literally means to cut the bone around the hip socket.
- A PAO is a surgical procedure where cuts are made in the pelvis bone, around the hip socket. The hip socket is then repositioned to provide the appropriate amount of coverage for the femoral head.
- Studies have shown that with the correct indications, PAO can delay the need for additional surgeries, such as a hip replacement, by an average of 28 years.
